Transfer Checklist & Documentation

1. Application Inquiry
Complete and submit the online Transfer Application form (located on this page).
2. Transcript Release Form
Complete the Transcript Release Form and submit it directly to your current school. Records must be sent to Our Lady of Tepeyac High School.
3. Academic Records
  • Official Transcripts from your current school (for the past 2 years, if applicable).
  • Report Cards from the most recent grading period.
  • Standardized Test Scores (PSAT, SAT, ACT, or achievement tests).
4. Immunization & Medical Records
  • Complete Immunization Records (including state-required vaccines).
  • Health/Physical Examination form.
  • Medical or Special Needs documentation for accommodations (if applicable).
5. Disciplinary & Attendance Records
  • Official Disciplinary Record from current school.
  • Official Attendance Records.
6. Special Education or Testing Information
  • IEP (Individualized Education Plan) or 504 Plan (if applicable).
  • Psychological Evaluation Reports (if applicable).

*A meeting with the student and parent/guardian will be scheduled once documentation is provided.

7. Parent/Guardian Information
  • Legal documentation (required if you are not the student's legal parent).
  • Emergency Contact Information (phone numbers and email addresses).
8. Financial Aid
Once accepted, families may apply for Financial Aid. Income documentation will be required.

9. Student Essay
Briefly summarize your reasons for wanting to transfer to Our Lady of Tepeyac High School.
Important Notes:
  • All required documents must be submitted directly to the Main Office.
  • Deadline: Submitted by the date of your scheduled appointment.
